Important note for women: If you are pregnant or trying to become pregnant, you must not dive.

Recreational scuba diving and freediving place physical and mental demands on the body. Certain medical conditions may increase risk while diving. If you have any health concerns not addressed in this questionnaire, or if you are unsure about your fitness to dive, we strongly recommend seeking advice from a qualified medical professional before participating. Do not dive if you are feeling unwell. If you believe you may have a contagious illness, please refrain from dive training or activities to protect both yourself and others.

For your safety, and the safety of those diving with you, all questions must be answered fully and honestly.

Have you ever suffered any illness or injury that may affect your ability to dive or snorkel safely?
Are you currently suffering or have ever suffered from any illness, injury or condition relating to a heart or lung disorder, asthma, epilepsy, diabetes or any condition that is contraindicative to diving?
Are you currently taking any prescription medication? (with the exception of birth control or anti-malarial medication)?
Is there any reason you are NOT medically or physically fit to dive or snorkel?
Could you be pregnant?
Do you have any questions or uncertainty about the risks of diving if a medical condition is concealed or undisclosed?
