Please complete this short self-assessment before your trip, to help us ensure your experience is safe and enjoyable.

 

To dive safely, you should be in good general health and comfortable in the water. You should not dive if you have heart or lung problems, a current cold or congestion, epilepsy, asthma, or any serious medical condition — or if you are under the influence of alcohol, drugs, or certain medications. If you are taking prescribed medication or have any medical concerns, please check with your doctor before joining this activity.

The purpose of this questionnaire is to identify whether you may need medical clearance before participating in scuba diving. A “Yes” answer does not automatically prevent you from diving — it simply means you should seek professional medical advice before entering the water.

Have you suffered from, or do you now suffer from any of the following?
Are you currently suffering from another illness or have had an operation within the last month?
Are you currently taking any medicines or drugs (excluding oral contraceptives)?
Do you understand that you should not go to altitude (fly) within 12 hours of completing a single dive or 24 hours when doing multiple dives?
Could you be pregnant?
Do you understand that any concealment of any condition incompatible with safe diving might put your health or life at risk?
